Job summary

ProMedica seeks a dedicated Registered Nurse to join our Center For Health Services in Toledo, OH. This role supports a patient-centric care delivery model within a physician office setting, navigating multiple IT platforms to uphold value-based care and ProMedica Core Values.

You will collaborate with providers and leadership to enhance patient experiences, prepare patients for visits, assist with clinical procedures, and coordinate referrals as needed.

Qualifications

• Current State license as a Registered Nurse.
• 1 year of relatable or equivalent experience.
• ACLS and BLS certification.
• BSN is preferred.

Responsibilities

• Works closely with the provider and leadership to continually improve the patient experience through daily patient care, clerical support, ongoing projects and assumes additional duties as needed self-directed.
• Prepares the patient for the visit by gathering baseline data, documenting reasons, obtaining vitals and office testing.
• Assists clinicians with treatments, dressings, and other tasks; sets up exam rooms and ensures needed equipment.
• Triage phone calls; coordinate care for patient with provider and other service lines/specialties; provide referrals as appropriate.
• Relates to patients with care and clear communication; documents encounters accurately and understands charting implications.
• May perform Lab functions and draws; assists leadership with ordering and monitoring office medications.
