Healthcare Network is currently hiring an OB Care Coordinator for the Population Health Team, located at 1454 Madison Avenue, Immokalee, FL. The role focuses on coordinating obstetric-related care, supporting patients with access to services and coordinating with clinicians to improve maternal health outcomes.

Healthcare Network (HCN) is a Federally Qualified Health Center (FQHC) serving as the medical home for patients from birth to geriatric. We provide quality medical, dental and behavioral health services through board certified physicians and evidence-based practices to ensure that the whole patient receives quality care, regardless of insurance or income status.
