Job summary

VNA Health Group, a leading home health and hospice provider in New Jersey, seeks an experienced RN to join our Coding Review team as Clinical Data Coordinator/OASIS Reviewer. You will oversee OASIS reviews, support ICD-10 coding, and ensure regulatory compliance while promoting documentation excellence across the organization.

Collaborate with clinical and operational teams to improve patient outcomes, participate in quality initiatives, and ensure adherence to Medicare regulations and care

Qualifications

• Current NJ RN License required.
• BSN preferred.
• OASIS Certification required or actively pursuing.
• Coding Certification preferred.
• Minimum 3 years of home health experience.
• Strong knowledge of OASIS, ICD-10 coding, and Medicare regulations.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work independently and manage multiple priorities.

Responsibilities

• Review OASIS assessments for accuracy, completeness, and regulatory compliance.
• Ensure documentation is consistent with the Plan of Care and medication profile.
• Submit OASIS data and resolve rejections or discrepancies.
• Identify trends and communicate findings to leadership and clinical teams.
• Provide ICD-10 coding support as needed.
• Ensure compliance with Medicare, Medicaid, federal, state, and agency regulations.
• Collaborate with clinical and operational teams to support quality patient outcomes.
• Review clinical documentation and recommend corrective actions when appropriate.
• Participate in quality improvement and educational initiatives.
