Job description

Client Care Coordinator

Responsibilities, not limited to:

1. Interview children and parents in shelter to complete an assessment and determine an appropriate plan of action;
2. Prepare children and parents to accept services;
3. Make referrals, when appropriate, to governmental or not-for-profit agencies;
4. Provide direct counseling services and/or outreach services; make referrals to other public agencies or community resources as needed;
5. Confer and consult with professional and technical personnel in implementing a multidisciplinary approach to client care and well-being;
6. Serve as agency/program liaison to community agencies and/or groups;
7. Provide clear written reports that capture family assessment findings and recommendations; complete all documentation in a timely manner;
8. Prioritize and visit the highest risk families in shelter to coordinate safety planning efforts;
9. Provide training, consultation, and coaching to shelter-based case managers.

Qualifications:

1. Master's/MSW and LMSW required within three months of hire.
2. Master of Social Work Degree from an accredited school of social work; current licensure as LMSW within three months of hire.
3. Knowledge in areas such as child and adolescent development, mental health, emotional/behavioral health, parent-child relationships, family dynamics, and diagnostic classification.
4. Strong expertise in strengths-based, solution-focused, and family-centered practices.
5. Knowledge of guidelines, policies, and regulations related to child welfare, safety, permanency, and well-being.
6. Proficiency in Spanish or French is a plus.
7. Experience working with diverse cultures and ethnicities.
8. Excellent written and verbal communication skills.
