Job summary

Cumberland County Department of Public Health seeks a Nutritionist II to plan, develop, and implement nutrition education services in-home and in the community. You will provide nutrition counseling, complete assessments, and work with WIC staff to ensure access to services.

Responsibilities include case management collaboration, health literacy considerations, and referrals to other programs. Must have or obtain required nutrition credentials and meet driving license requirements.

Qualifications

• Registered Dietitian (RD) with the Commission on Dietetic Registration (CDR) or eligible for RD credentials.
• Master's degree in Dietetics, Public Health Nutrition, or Nutrition.
• Completion of CADE-accredited Didactic Program in Dietetics and two years nutrition experience.
• Dietetic Technician, Registered (DTR) with relevant bachelor’s degree and two years nutrition experience.
• Bachelor’s of Science in Dietetics, Public Health Nutrition or Community Nutrition with two years nutrition experience.

The Cumberland County Department of Social Services (DSS) and the Cumberland County Department of Public Health (CCDPH) have received funding to implement an initiative which focuses on primary prevention by utilizing a whole person/family approach and addressing social determinants of health (SDOH). The initiative includes multi-disciplinary Case Management and Care Coordination teams responsive to community referrals to provide care coordination and case management services to address social determinants of health, address social needs, improve health behaviors and health outcomes, improve health equity, and reduce health disparities, reduce preventable hospital stays, and prevent and reduce child abuse and neglect.

The Nutritionist II is an in-home and community-based based position. This position will plan, develop, and implement nutrition education services, including nutrition and breastfeeding support. The Nutritionist II will provide nutrition counseling and education, complete nutrition assessments, and provide nutrition instructions to patients. The position will work collaboratively with Cumberland County WIC staff to assure access to WIC services.

The duties for this position include, but are not limited to:

• Serve on a multi-disciplinary Case Management and Care Coordination Team with a Health Educator, Social Worker, Nurse, and an Income Maintenance Worker) provide case management and care coordination services in-person (in home and office), by telephone or virtually.
• Implement nutrition education services to Health Department patients and Department of Social Services Clients with consideration for health literacy, health equity, and cultural competency.
• Assess the ongoing nutrition education and outreach needs of individuals and families enrolled in the Case Management and Care Coordination Program.
• Identify resources needed to implement assessed nutrition education and outreach needs.
• For breastfeeding individuals, assess the need for and provide counseling and/or caregivers counseling regarding the need and proper use of special formulas and supplements.
• Using division procedures/protocols, make any needed referrals identified during screening/assessment to other agency division/programs and medical provider offices.

Employee is subject to be called upon anytime to help the agency respond to a public health emergency which may include, but is not limited to, outbreak investigations, staffing mass dispensing or immunization clinics, providing disaster relief, or other incidents (man-made or natural and intentional or non-intentional) as they arise, or any public health response requested by the Health Director.

or a Bachelor's of Science degree in Dietetics, Public Health Nutrition or Community Nutrition from an accredited four-year college or university and two years nutrition experience

Administering the Class-

Candidates should provide documentation of registration status from the Commission on Dietetic Registration; provide university or college transcripts or CADE verification statement.

Degrees must be received from appropriately accredited universities.

• Considerable knowledge of the principles of normal and therapeutic nutrition.
• Considerable knowledge and skill in nutrition and dietary assessment techniques (anthropometrics, biochemical, clinical and dietary), and in interpretation of data.
• Knowledge on interrelationships between health and social service programs, and of appropriate community resources for referral of clients.
• Knowledge of human behavior and techniques for effecting behavior change.
• Skill in writing clear concise entries using the protocol for progress notes in clients’ health/medical records, and the ability to interview and provide appropriate counseling.
• Ability to understand an individual’s socio-economic status and lifestyles with respect for individual differences.
• Ability to monitor and maintain quality standards of nutrition care using current standards of practice.
• Ability to organize and manage time and resources.
• Ability to make accurate observations and report changes accurately, to remain calm and function appropriately and efficiently in emergency and stressful situations.
• Ability to understand, interpret and relate oral and written instructions, and to be able to establish and maintain effective working relationships with others.

Must have or obtain and maintain a valid North Carolina driver's license with an acceptable driving record. Background check and negative drug test are required.
