Position Summary

The Nutritionist/Dietician provides high-quality nutrition education, counseling, and intervention for patients in an ambulatory care setting. This role is essential in chronic disease management, diabetes education, and preventative health, empowering patients with the knowledge and tools needed to make informed dietary choices that support long-term wellness. The role involves direct patient care, program management, research participation, and interdisciplinary collaboration to improve overall health outcomes and enhance the quality of life for patients.

Core Job Responsibilities

1. Educate patients and families on disease prevention, chronic disease management, and nutrition-related disorders in an outpatient setting.
2. Assess patients’ nutritional needs, create tailored meal plans, and coordinate care with physicians, diabetes educators, and healthcare teams.
3. Ensure the program meets the American Diabetes Association (ADA) recognition standards, with accurate documentation and compliance.
4. Perform other responsibilities as assigned.

Requirements/Preferences

1. Bachelor’s degree required. Master of Science in Dietetics or Nutrition strongly preferred.
2. Valid Illinois license as a Registered Dietitian required.
3. Certificate Diabetes Educator (CDE) preferred.
4. Minimum of two (2) years’ experience as a certified diabetes educator preferred.
5. Experience in an outpatient diabetes center strongly preferred.
6. Experience with American Diabetes Association (ADA) recognition standards strongly preferred.
7. Bilingual (English/Spanish) required.
8. Proficiency in Electronic Health Record (EHR) and EPIC preferred.
9. Intermediate proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).

Working Conditions

1. Normal office and health center environment regularly.
2. Local travel between health centers and community events is required; mode of transportation required.

ACCESS is a Network of Federally Qualified Health Centers treating patients on the frontlines of community-based health care. Depending on the position applied for, candidates may be required to be vaccinated against communicable diseases and provide supporting documentation proving that they are properly vaccinated, or apply for religious and/or medical vaccination exemption as a part of the application process.

The pay ranges provided represent the minimum to mid-range for positions. Actual compensation will be determined based on a combination of factors including years of experience, educational background, market conditions, and available grant funding.
