Join to apply for the Nutrition Educator-Remote role at Mom's Meals | A Purfoods Company

The Nutrition Educator will provide telephonic nutrition education to members/patients with chronic conditions as they learn to select, prepare, and eat foods appropriate for their conditions, participating in a chronic care meals program. Chronic conditions can include, but are not limited to, diabetes, heart disease, and kidney disease. Utilizing a member/patient-centric approach, the nutrition educator will collaborate with the member/patient to explore nutrition-related issues and topics most relevant to them. The educator will also connect members/patients to other nutrition and condition resources available from the health plan, system, or community.

Salary range: $55,388 - $72,000 plus bonus; one opening posted in multiple locations for licensure needs.

Responsibilities may include, but are not limited to:

• Providing telephonic nutrition education sessions based on the member/patient's enrolled program.
• Documenting goals and care plans in the platform.
• Scheduling follow-up calls.
• Developing nutrition education handouts and tools as needed.
• Identifying community resources for clients.
• Maintaining professional knowledge through educational opportunities and networks.
• Performing other nutrition-related tasks as needed.

Required skills and experience:

• Bachelor of Science in Nutritional Science or Dietetics.
• Registered Dietitian certification by the Commission on Dietetic Registration.
• At least 2 years of experience in nutrition education or counseling.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work independently with minimal supervision.
• Proficiency with standard office equipment.
• Ability to make clinical decisions regarding nutrition education appropriateness.

Preferred skills and experience:

• Active licensure in District of Columbia or Ohio.
• Experience with telephonic nutrition education.
• Training in health coaching, motivational interviewing, or mindful eating.

Physical requirements:

• Repetitive motions involving wrists, hands, or fingers.
• Sedentary work involving prolonged sitting.
• Visual perception skills for job performance.
