We are hiring a 7a-3p Unit Manager to join our dynamic care team at Cape Heritage Rehabilitation & Health Care Center in Sandwich!

As the Unit Manager, you will be responsible for the 24-hour coordination and management of resident care activities, unit operations, and nursing personnel on a designated nursing unit or shift, in accordance with current rules, regulations, and guidelines that govern the long-term care facility.

Education & Experience:
  • Must possess an active and unencumbered RN or LPN license in Massachusetts.
  • Bachelor's in Nursing preferred; CPR certification required.
  • At least three (3) years of experience, including one year in long-term or subacute care.
  • One year of supervisory experience preferred.
Duties & Responsibilities:
  • Serve as a resource to staff in clinical and managerial decision-making, delegate responsibilities appropriately.
  • Assist in coordinating nursing services with other resident care services to ensure continuity of care.
  • Participate in inspections (surveys) by authorized government agencies.
  • Participate in and validate residents' assessments and nursing staff’s clinical decisions.
  • Develop, implement, and communicate residents' care plans.
  • Ensure staff implements care plans correctly and recognizes changes in resident status.
  • Conduct regular rounds to ensure staff performance and resident needs are met.
  • Monitor documentation procedures, audit daily to ensure compliance with federal, state, and facility requirements, and follow up on issues.
  • Collaborate with other departments to meet resident needs.
  • Recognize staffing needs within established patterns and facilitate appropriate staffing.
  • Hold regular meetings with staff to address issues and improve services.
  • Provide direct care or function in a staff nurse role as needed.
  • Follow universal precautions and infection control policies.
  • Review and address complaints and grievances, reporting actions taken to the Director of Nursing.
Specific Requirements:
  • Ability to read, write, speak, and understand English.
  • Accurate assessment skills.
  • Knowledge of drug actions and interactions.
  • Ability to interact tactfully with personnel, residents, families, visitors, and agencies.
  • Leadership and supervisory skills, ability to work harmoniously with staff.
  • Ability to plan, organize, develop, implement, and interpret care programs and policies.
