Qualifications

• Must be a Registered Nurse with a current Indiana license.
• At least 1 year of supervisory experience.
• Good communication is essential for documenting care and collaborating with the team.
• Valid driver’s license required and background clearance is part of onboarding.

Responsibilities

• Develop, manage, and grow Wellness Coordination services.
• Supervise and manage nursing staff in a select area.
• Ensure Wellness Assessments, Risk Mitigation Tools, and Wellness Plans are completed for each individual.
• Ensure face-to-face visits are documented as outlined in policy.
• Provide leadership to promote the organization’s mission and ensure quality health care services.
• Offer nursing consultation to area and program directors for appropriate services.
