POSITION SUMMARYResponsible for providing direct resident care as defined by the job description and as directed by supervisor.Reports to the Charge Nurse, Unit Manager, Evening House Supervisor, Director of Nursing/Director of Resident Health Services, and Administrator.Performs all duties in compliance with established facility policies and procedures.ESSENTIAL FUNCTIONSProvide care for and assistance with Activities of Daily Living (ADLs) to residents as needed or as directed, including but not limited to bathing, dressing, feeding, personal hygiene, toileting, positioning, and mobility; taking all necessary measures to ensure the highest level of comfort, dignity, and safety to the resident at all times.Report any changes in condition, including any evidence of skin breakdown or wounds, incidents, accidents, and unusual events to supervisor immediately.Obtain vital signs and record as directed.Review Daily Assignments at the start of each shift, noting any changes in care or transfer status.Promptly record all care provided, using electronic medical record or paper chart as appropriate.Prepare residents for meals and/or snacks, transporting to and from dining areas as needed. Assist with serving, identify food arrangement for sensory impaired residents, and assist with feeding as needed. Record food and fluid intake for each meal and/or snack as appropriate.
