Job summary

Claratel Behavioral Health seeks a RN leader to direct nursing activities for Developmental Disabilities (DD) Services, including supervision of RNs, LPNs and Med Techs in community-based settings. You will set nursing standards, develop staffing plans, and drive quality improvement across DD programs.

The role requires GA RN licensure, a BSN, and at least 2 years of supervisory experience with DD or similar populations.

Job description

Plans, organizes, and directs all nursing activities for Developmental Disabilities (DD) Services (Community based residential and Community day services). Provides leadership to Licensed Practical Nurses (LPNs) and medical support staff. Coordinates response to urgent and emerging medical needs. Contributes to the development of policies, sets standards and plans objectives for the DD program relating to nursing in accordance with established policies. Develops staffing patterns for homes where LPN services are needed. Evaluates nursing standards and nurse protocols for DD programs. Interprets policies and communicates changes in regulations and procedures. Makes long-range plans and regularly reviews activities, problems and functions of nursing services being delivered. Organizes and plans a systematic and continuous quality/performance improvement program for the practice of nursing. Plans and directs the work of nursing or other professional staff and assures an adequate level of nursing care. Provides or ensures staff development including orientation, in-service training and continuing education for all levels of DD staff as it relates to medical services. Ensures that persons served receive the best possible healthcare by providing overall monitoring of health indicators as well as identifying and advocating for healthcare needs and services. Provides consultation and direction to the support services team regarding the health concerns of people served and advises agency management on the development of internal protocols that support the best possible healthcare outcomes. Develops and implements formal training programs that provide support service staff with the knowledge & skills required to protect the health and well-being of individuals and that comply with licensing standards & best practice.

DUTIES AND RESPONSIBILITIES
  • Oversees all aspects of the healthcare needs of service recipients in Developmental Disability Services.
  • Supervises Registered Nurses (RNs), LPNs and Medical Technician (Med Tech).
  • Completes all required training with new RNs, LPNs and Med Tech. Ensures that RNs, LPNs and Med Tech meet annual training and licensure renewal requirements.
  • Completes monthly LPN schedule for applicable residential homes.
  • Completes annual evaluations for assigned medical/nursing staff.
  • Reviews medical appointment documentation (including but not limited to: urgent, emerging, routine and follow-up medical care, routine lab-work) ensuring that all recommendations and orders are communicated to the appropriate staff and ensures all orders are carried out.
  • Ensures that individuals obtain an annual physical exam, periodic dental exams, as well as other routine and follow-up medical care such as routine lab-work by providing supervision and direction to the Med Tech who schedules and takes individuals on appointments.
  • Directs the effort for Health Risk Screening Tool (HRST) completion by RNs, LPNs, Med Tech and Developmental Disability Professionals (DDPs) for all DeKalb CSB assigned individuals by using HRST logs, medical visit reports, incident reports and other pertinent medical information.
  • Reviews and signs any HRST with a rating over 3.
  • Reviews HRST tracking for any individual with a HRST over 3.
  • Completes and/or ensures completion of Crisis Safety plan, when applicable. Ensures Crisis Safety plan is current, communicated and signed timely by appropriate medical personnel.
  • Ensures healthcare plans and/or health risk plans, when applicable, are current, communicated and signed timely by appropriate RN.Coordinates efforts to ensure that all medical professionals who provide services to the person served have all pertinent medical information regarding the person such as current lab-work and medications.
  • Coordinates the annual PPD testing and annual flu shots (if applicable) in all DD programs.
  • Coordinates discharge planning with hospitals and other medical facilities for persons supported in the DD residential program.
  • As applicable, provides DD staff with feedback regarding medical appointments and doctor’s orders.
  • Ensures monthly monitoring of weight logs for pertinent changes in weight for DD Residential individuals and provides direction for follow-up to applicable medical staff, residential staff and pertinent team members. Sends out monthly weights to homes.
  • Ensures Nursing staff attends ISP meetings for all persons in residential services who have precarious medical statuses.
  • Completes the upcoming monthly Medication Admiration Records (MAR) by the last day of the month.
  • Coordinates effort by LPNs to update MARs with all medication additions or changes as ordered throughout the month and ensures that these changes are communicated to home (and day program as applicable) and then reflected on HRST.
  • Coordinates nutritional consults as ordered by Personal Care Physician (PCP). Coordinates follow-up on nutritional assessments to ensure compliance with doctor’s orders.
  • Completed weekly visits to homes supporting individuals with exceptional rate medical needs and completes required documentation.
  • Provides training to CNAs or comparable staff working in Community Living Arrangements/Personal Care Homes as needed.
  • Provides/ensures Proxy training to new hires and current staff as needed. Proxy training will include Medication Administration, Glucometer reading, G-tube flushing, and other agency approved Health Maintenance Activities. Coordinates effort for the Proxy Plan of Care renewals to be signed by agency contracted RN completing training (when completed by contracted RN) and by the individual’s PCP.
  • Develops policies and protocols for medical supports and training in conjunction with DD management as needed.
  • Provides 24/7 medical support through their own actions or the actions of DeKalb Community Service Board (DCSB) RNs/LPNs or proxy caregivers

Provides necessary medical direct case as needed

SKILL, COMPETENCY AND EDUCATION AND/OR LICENSURE REQUIREMENTS

Registered Nurse with current valid RN license in the state of Georgia and a bachelor’s degree in Nursing from an accredited school. (Masters preferred)

Minimum 2 years’ experience serving individuals with developmental disabilities or other group with similar needs, as well as at least two years supervising other staff.

Possess and demonstrate well-developed communication skills and a sympathetic disposition.

Ability to develop and maintain constructive and cooperative working relationships with others.

Strong ability to organize, plan, and prioritize in order to accomplish relevant or assigned work responsibilities.

WORKING CONDITIONS

Maintains ongoing communication and makes onsite visits as necessary to all DeKalb CSB DD programs. On call 24/7, directly or indirectly through subordinate staff, for medical emergencies and medical support. Office hours 8:30am-5pm, Monday-Friday with 30 minute lunch, but might be subject to change based on programmatic needs.
