Job summary

Hospice Buffalo seeks a Home Care Licensed Practical Nurse to provide skilled nursing care in patients' homes under the plan of care. You will document per policy, educate patients and families, and coordinate care with other health professionals.

The role requires NY LPN licensure, a valid driver's license, and about 1 year of nursing experience. Travel within the community is expected, with a stable full-time schedule and a strong rewards package.

Qualifications

• Graduate from an accredited School of Practical Nursing with current LPN licensure in New York.
• Valid Driver's License and vehicle required for on-call or on-duty use.
• 1 year of nursing practice as an LPN preferred.

Responsibilities

• Provides skilled nursing care to patients under the plan of care and agency policies.
• Documents timely and accurately per policy and HIPAA guidelines.
• Reinforces patient and family teaching about disease, self-care, and prevention.
• Participates in planning and coordination with other health professionals.
• Assists physicians and RNs with procedures and on-call visits.
