Job summary

Community Health is seeking a Nursing Home Care Coordinator to assist providers by handling nursing and administrative duties, ensuring safe and efficient care for patients across multiple locations in Rutland and southern Addison counties.

You will coordinate orders, triage calls, record vitals, preload admissions, and maintain accurate patient records while collaborating with nursing staff to meet regulatory and payer requirements.

Qualifications

• CCMA preferred, CPR certification.
• Prior nursing home experience preferred.
• Experience with electronic medical records and ability to learn other systems.
• Basic keyboarding skills and email communication.

Responsibilities

• Communicates provider orders and updates SNF staff on new orders.
• Triages calls based on urgency and refers complex issues to the provider.
• Obtains and records vital signs using electronic health records.
• Pre-loads new patients for admissions and updates patient records.
• Documents and maintains nursing home data for measured health metrics.
• Schedules regulatory visits and tracks documentation.

ABOUT THE ROLE:

The Community Health Nursing Home Care Coordinator will assist the providers in managing an optimal workflow by providing a variety of nursing and administrative duties to provide safe and efficient care of the patients.

FUNCTIONS OF THE POSITION:

• Communicates with provider about any new orders on patients and communicates to the appropriate SNF nurse that there are new orders placed, the indication for those orders while fielding subsequent calls to answer all questions from the nursing staff.
• Assists in phone outreach to include by not limited to handling caller needs/ directing to appropriate staff, triage calls which will be based on urgency and severity of symptoms and refers to provider for complicated issues.
• Obtains and records vital signs utilizing multiple electronic health records.
• Pre-loads all new patients for admissions and updates current Community Health patient records.
• Documents and captures all pertinent nursing home data for all Community Health measures.
• Maintains, tracks, and schedules all regulatory visits.
• Addresses daily summary action report to ensure Community Health patient records are correct and update to date with new payor sources.

SKILLS REQUIRED FOR SUCCESS:

• CCMA Preferred, Valid CPR Certification.
• Prior experience working in a nursing home preferred.
• Experience in using a variety of electronic medical record and ability to learn other systems, basic keyboarding skills and email communication.
