Qualifications

Current RN license in Illinois.
BSN degree required; MSN preferred.
Minimum of 3 years in home health care with at least 1 year in a leadership or management role.

Responsibilities

Oversee the nursing department, including staffing, training, and performance evaluations.
Develop and implement policies and procedures to ensure quality care and compliance with local, state, and federal regulations.
Collaborate with interdisciplinary teams to develop and evaluate patient care plans.
Monitor patient outcomes and implement quality improvement initiatives to reduce rehospitalizations.
Provide mentorship and support to nursing staff, fostering a culture of continuous learning and professional development.
Serve as a liaison between patients, families, and healthcare providers.
Ensure accurate documentation and maintain patient records using WellSky Kinnser EMR in accordance with HIPAA regulations.
