InfuCare Rx is seeking a Specialty Pharmacist to oversee the infusion suites, coordinate patient care, and support home infusion patients. You will collaborate with physicians, nurses, and staff to ensure safe, effective drug therapy and accurate documentation.

The role requires a PharmD or BS in Pharmacy, state licensure, and at least two years of home infusion experience. Flexibility with schedule and willingness to obtain additional licenses are essential.

Qualifications

  • Bachelor of Science in Pharmacy (RPh) or Doctor of Pharmacy (PharmD) from an accredited college.
  • Licensed as a registered pharmacist in the state of employment.
  • Willing to obtain out-of-state licenses as requested by InfuCare Rx.
  • Minimum two years of recent experience in home infusion therapy, focusing on specialty infusion medications.

Responsibilities

  • Oversee Infusion Suite operations and ensure safe, effective drug therapy.
  • Review and manage specialty medications for complex/high-cost conditions.
  • Coordinate referrals/admissions with nursing, customer service, and distribution.
  • Review and verify new patient orders and document accordingly.
  • Monitor therapeutic outcomes via labs and clinical data; report results to physicians and nurses.

Overview

As a Specialty Pharmacist dedicated to oversite of InfuCare Rx's infusion suites, you will be involved with multiple facets of the Infusion Suite Experience for our patients, caregivers and providers. Additionally, you are expected to provide support for our home infusion patients. This will involve your interaction with insurance companies, staff members, distributors, Physicians/Nurses and patients to ensure accurate and safe quality service. You will use your experience as part of the Pharmacy Team to resolve problems when unforeseen roadblocks arise. Perform specific distributive and clinical pharmacy activities and related services as they pertain to providing safe and effective home infusion therapies.

General Duties:
  • Adheres to organization policies and procedures.
  • Reviews and manages specialty medication therapies for complex, chronic, or high-cost conditions.
  • Coordinates and facilitates new referral/admissions through collaboration with nursing, customer service, and distribution.
  • Reviews, receives/verifies all new patient orders and documents on the order form.
  • Assists the physician and other healthcare professionals involved in planning and implementing patient care in the home / suite to ensure safe and effective drug therapy.
  • Conducts initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug-related problems or needs.
  • Assists the physician and nursing staff in setting therapeutic objectives for drug therapy and establishing parameters for monitoring the safety and efficacy of drug therapy.
  • Monitors the outcomes from drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information.
  • Communicates the results of therapeutic drug monitoring activities to the physician, nursing and other appropriate persons.
  • Programs infusion control devices as indicated.
  • Provides consultation and answers inquiries from patients, families, physicians, and other healthcare professionals regarding drug and their use.
  • Maintains thorough and appropriate documentation in the patient/client record of all clinical activities and communication with patients, physicians, and other healthcare professionals.
  • Participates in the preparation of progress reports for the referring physician on a regular, periodic basis.
  • Supervises, coordinates and/or participates in the preparation of sterile admixtures, dispenses all pharmaceutical and related supplies and equipment in accordance with federal, state and local laws and standards of practice which govern the profession of pharmacy.
  • Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.
  • Maintains pharmacy records in accordance with applicable laws and regulations.
  • Maintains clinical skills and knowledge of pharmacotherapy by reviewing the literature and attending in-services, seminars and conferences.
  • Participates in on-call rotation if applicable.
  • Performs marketing support functions as requested.
  • Participates in orientation/training programs for new personnel and current staff.
  • Participates in departmental QA activities.
  • Attend designated department meetings as scheduled.
  • Performs other tasks or special projects as requested by management.
Confidentiality and Compliance
  • Maintain confidentiality of patient and proprietary information at all times.
  • Observe legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary company information.
  • Understand and comply with all HIPPA requirements and regulations
  • Participate in the process for assessing, managing, and reporting medication-related incidents and equipment error and/or failures. Accurately completes occurrence forms and forwards them to the appropriate governing bodies.
  • For all activities comply with accreditation, legal, regulatory and safety requirements.
Qualifications: (Education, Certification/licensure and Experience/Skills)
  • A Bachelor of Science in Pharmacy (Rph) or Doctor of Pharmacy (PharmD) from an accredited (Council on Pharmaceutical Education) college of Pharmacy.
  • Currently Licensed as a registered pharmacist in the state of employment.
  • Must be willing to obtain out of state pharmacist licenses as requested by InfuCare Rx Licensing Team.
  • Possess a minimum of two years' r recent experience in home infusion therapy, with focus on specialty infusion medications.
  • Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses and other healthcare professionals.
  • Can work effectively with others to provide safe and effective drug therapy.
  • Can communicate, both verbally and written, with patients, clients, management, and other employees.
  • Can problem solve and supervise technical and other support staff.
  • Must be flexible with work schedule.
  • Possess good organizational and time-management skills.
  • Participation and membership in professional societies and organizations is desirable.
  • Dependable in attendance and job performance.
Physical Demands:

Required to stand, walk, sit, talk, and hear; required to use hands to operate vehicles and office equipment; must be able to occasionally lift and/or move up to 25 lbs; specific vision requirements for this job include: close, distance and peripheral vision, and the ability to adjust focus, reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position without compromising patient care.
