Responsibilities include care planning, documentation, and monitoring health outcomes while traveling up to 25% for community or home visits.

About The Health Care Home Program
Our Health Care Home team supports preventive care, chronic condition management, and health education for individuals with complex physical and behavioral health needs, including children and adolescents.
As a Nurse Case Manager, you will play a central role in coordinating care, improving health outcomes, and supporting clients through proactive outreach and structured engagement. This is a highly collaborative role, working closely with Community Support Specialists and interdisciplinary care teams.

This role includes structured expectations to support consistent client engagement and high-quality care delivery. Nurse Care Managers are expected to maintain regular outreach activity, complete timely documentation, and contribute to defined program productivity standards that are both achievable and supported by the team.
