Salary: Starting at $75000 / annually - depending on education / experience.

Job Summary:

The Nursing Care Manager will oversee and coordinate comprehensive nursing care to ensure optimal resident outcomes. This role involves shift to shift report with unit nurses, managing physician orders, conducting initial assessments for new admissions, attending pre-placement meetings, following up on external medical visits, and responding to on-campus medical emergencies. The Nursing Care Manager will follow clinical protocols and maintain accurate medical records of each resident on their caseload.

  • Unit Nurse Assignments: Assigns daily clinical tasks to unit nurses, ensuring assignments are posted and updated promptly. Follows up on residents with new illnesses to ensure documentation has been completed.
  • Physician Order Management: Accurately enters and processes physicians’ orders into the electronic health record system. Follows up on refill requests and the need for prescriptions from RL health and CBH providers.
  • Patient Assessment and Documentation: Performs comprehensive initial assessments for new admissions, including medication reconciliation, nursing notes, and overall health evaluations.
  • Interdisciplinary Collaboration:
    • Participates in pre-placement meetings to discuss and plan patient care strategies for new admissions
    • Collaborates with case management to address parent/family concerns
  • Continuity of Care: Monitors and follows up on patients’ hospital visits and consultations with external specialists to ensure seamless care transitions. Calls resident families to provide updates on new orders, hospital visits, and family requested follow-up calls
  • Medication Management: Addresses pharmacy-related issues and ensure medication orders in the Quick MAR system are current and accurate.
  • Appointment Coordination: Works closely with the RL Health Nurse Navigator, Unit Nurse or designee to schedule same-day appointments as needed. Collaborates with Medical Schedulers regarding outside appointment need and follow up requests
  • Clinical Assessments: Oversees and tracks the annual completion of the Glasgow Coma Scale and bi-annual completion of the Abnormal Involuntary Movement Scale (AIMS) assessments for applicable residents.
  • Emergency Response: Responds promptly to medical emergencies on campus, providing immediate nursing care and coordinating further medical intervention when necessary
  • Lifetime Medical Summary: Ensures lifetime medical records are updated and maintained in compliance with regulatory standards. Including Input of new data into Insync.
  • Performs other duties as assigned to support the department and enhance resident care.
  • Licensure: Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state
  • Experience:
    • Minimum of 3 years of nursing experience, preferably in residential or similar healthcare setting.
    • Strong organizational and multitasking abilities.
    • Excellent communication and interpersonal skills.
    • Proficiency in electronic health record systems and medication management software.
    • Ability to work collaboratively within an interdisciplinary team.
    • Critical thinking and problem-solving skills.
  • Physical Requirements:
    • Must be able to lift a minimum of 50 pounds, be agile, physically pursue clients on foot, physically restrain, bend, squat, stretch, reach, push up and stand for several hours at a time.
    • Capacity to respond swiftly in emergency situations.
