Who You Are

We are seeking a candidate with a strong positive attitude toward customer service to act as an LVN in our Healthcare Facility. This person will have a genuine interest in working with older adults and be able to engage, motivate, and support both residents and fellow team members. Essential Functions
  1. Provide daily supervision to assigned staff and directly provide nursing care to residents in healthcare.
  2. Assesses the resident upon admission, discharge, transfer, change of condition, and on an ongoing basis for nursing needs and completes required record‑keeping forms/charts within the LVN licensure standards. Initiates the individual plan of care based on established health care goals, physician’s orders and nursing assessments with input from residents, guardians, family members or other responsible parties to establish goals to achieve the resident’s optimal level of functioning. Orients new residents/family members as necessary.
  3. Assists residents with Activities of Daily Living (ADL), family/social activities and actively promotes independent lifestyle of residents.
  4. Responds to residents’ call system promptly and courteously.
  5. Responsible for identification of resident medical and behavioral changes and assisting with development and monitoring of care plans and documenting response of each resident to care provided.
  6. Provides daily report to the Director of Nurses and/or Charge Nurse concerning community conditions, associate performance, resident conditions, and needs for nursing supplies.
  7. Consistently demonstrates hospitality behavior and the core values of quality, respect, service and integrity.
  8. Receives verbal orders from attending/alternate physicians, charts on physicians order sheet and follows established procedures to facilitate timely implementation of physician orders, notifying physician of auto‑stop orders on any medications and lab results.
  9. Notifies the attending physician and supervisor/administrator when a resident is discharged without approval. Assures that appropriate documentation is made in accordance with established policies and procedures.
  10. Reports all discrepancies of physicians’ orders, diet and charting to Director of Nursing.
  11. Notifies physician, family/responsible party, and DON/Executive Director of resident, family, or staff incidents. Completes appropriate resident assessments, resident, visitor/family, or employee incident report forms, and charts in the resident medical record on all such occurrences in accordance with facility policy and procedure.
  12. Provides nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, and administrative policies.
  13. Ensures preventive, therapeutic and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines.
  14. Makes rounds with physicians, assists in examinations and/or treatment of residents and carries out orders.
  15. Assures residents rights to fair and equitable treatment, determination, individuality, privacy, property, and civil rights, including the right to wage a complaint, are followed by all department personnel.
  16. Accurately administers prescribed medications, supervises the continuity of nursing care, responds to changes in medical condition, medications or other circumstances requiring special care needs and performs first aid as required.
  17. Provides daily supervision and leadership to assigned staff and other designated personnel in meeting the resident’s needs. Demonstrates good decision‑making skills. Provides written evaluation and oral counseling of assigned staff.
  18. Evaluates and monitors complex resident health status and determines the appropriate nursing intervention. Assists with developing and monitoring the ongoing plan of care with resident and initiates corrective action as necessary.
  19. Orients residents and associates to emergency procedures and responds to all emergency situations.
  20. Immediately reports to supervisor or initiates investigation on any reports of alleged abuse/misappropriation of resident belongings.
  21. Assures that all resident records are accurate, complete and all mandatory forms are on file. Through familiarity and application of community policies and procedures, assures that nursing care is practiced in full accordance with regulatory requirements, facility quality standards, and resident rights.
  22. Monitors resident environment to ensure all safety and sanitary regulations and quality standards are met or exceeded. May assist with development and presentation of in‑service programs.
  23. Assigns and directs specific tasks to other care providers to insure effective implementation of plan of care.
  24. Creates and maintains an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit.
  25. Develops and maintains a good working rapport with inter‑department personnel and other departments within the community to assure that department services and activities can be properly maintained to meet the needs of the residents.
  26. Assumes the authority, responsibility, and accountability of directing the unit/shift assigned.
  27. Assures that all personnel follow established infection control, aseptic techniques, and lifting techniques.
  28. Assures that the unit and resident room areas are maintained in a clean and safe manner for resident comfort and convenience. Assures that necessary equipment and supplies are properly maintained to perform such duties/services.
  29. Attends and participates in continuing education courses and facility in‑service training programs to assure that you keep abreast of current regulations, guidelines, professional standards, as well as facility policies.
  30. Maintains confidentiality of all pertinent resident care information and employee information.
  31. Follows instructions given and completes assignments according to directions and within the limits established. Knows when to ask for advice and accepts and follows suggestions from supervisor on how to improve work performance.
Qualifications
  • Licensed Vocational Nurse with a current State of Texas license.
  • Must be able to read regulations and write clearly.
  • Must be able to communicate in courteous, friendly manner.
  • Must be able to evaluate and make decisions in emergency situations such as fire, tornado, and other disasters.
  • Must be capable of functioning independently, have flexibility, personal integrity, and the ability to work effectively.
  • Possesses the ability to deal tactfully with personnel, residents, family members, visitors, governmental agencies/personnel and the general public in all circumstances.
  • Must report for duty at the scheduled time.
  • Ability to effectively manage and direct subordinates.
