    *must be external candidate to qualify; Sylvania outpatient hospice location only.

    Direct patient care under the direction of a RN or an LPN.

    ACCOUNTABILITIES

    1. Provides direct patient care as delegated by the R.N. per the plan of care and consistent with hospice policies, procedures, standards of care, and regulations.

    2. Responds to request for assistance in performing patient care functions within scope of STNA/CNA according to the Hospice Aide Plan of Care and consistent with state regulations for STNA/CNA certification.

    3. Monitors patients and communicates findings within scope of STNA/CNA according to the Hospice Aide Plan of Care and consistent with state regulations for STNA/CNA certification to RN or LPN in a timely and confidential manner.

    4. Accepts responsibility for care given and/or procedures performed as per policy.

    5. Documents care given or procedures performed on appropriate patient care sections in Epic.

    6. Maintains clean and safe patient care environment

    7. Communicates changes in patient condition or emergency situations notifies appropriate staff. Takes appropriate action immediately.

    8. Operates equipment safely. Identifies potential hazards and reports in a timely manner to appropriate staff.

    9. Demonstrates service standards and cooperation to patients, families and other co- workers.

    10. Performs other direct and indirect patient care functions as identified in the Hospice Aide Plan of Care and appropriate tasks delegated by the RN Case Manager.

    REQUIRED QUALIFICATIONS

    Education: High school diploma or GED

    Skills: BLS

    Years of Experience: 1 year (and/or 1040 hours) of patient care experience within past 24 months

    Certification: State Tested Nursing Aide (OH)- must be in good standing in State registry

    PREFERRED QUALIFICATIONS

    Years of Experience: Hospice Experience strongly preferred

    License: Certification in Ohio and Michigan preferred

    Certification: Certified Hospice and Palliative Nursing Aide (CHPNA)

    WORKING CONDITIONS

    Personal Protective Equipment: gloves, gowns, masks, eye shields

    Physical Demands: Must be able to lift medium materials, Non-material handling of: continuous standing, walking, bending, stooping and reaching; frequent pushing, pulling and squatting. Occasional sitting, stairs and kneeling.
