Responsibilities

• Performs personal care/interventions as assigned.
• Distributes water, snacks, and patient trays; assists with grooming and hygiene.
• Assists with ambulation, transfer, positioning and ROM; supports fall prevention.
• Collects and documents vital signs and health observations; reports changes to RN.
• Assists RN with admission, transfer, discharge and post-mortem care.
• May transport medications within the site and document accordingly.

This includes performing personal care such as feeding, bathing, dressing, grooming, moving and transporting patients, or changing linens.
