Responsibilities

• Assist with daily living activities (bathing, dressing, grooming, feeding, toileting).
• Monitor vital signs and record temperature, pulse, respiration rate, and blood pressure.
• Assist with mobility and transfers, help with walking and exercise as appropriate.
• Provide comfort and emotional support to patients and families; assist with safety measures.
• Observe and report changes in patients' physical, mental, and emotional condition to the nurse.
• Perform oral, hair, peri care, and catheter care.
• Weigh patients and document weights.
• Answer call lights promptly and position within reach.
• Document and maintain patient records and care activities.
• Maintain a clean, safe environment; change linens and clean rooms; ensure bed alarms are in place.
• Stock and replenish medical supplies in patient rooms and other areas.
• Assist with procedures and support nursing staff or provider as directed.
• Transport patients using lift equipment and assist with movement throughout the hospital.

Responsibilities

• Assist with Daily Living (ADLs). This includes helping patients with bathing, dressing, grooming, feeding and toileting.
• Monitor vital signs and record temperature, pulse, respiration rate, and blood pressure.
• Assist with mobility, helping patients move, including transferring them between beds, wheelchairs, and other areas, and assisting with walking and exercise when appropriate.
• Provide comfort and support, develop nurturing relationships with patients and families, and offer emotional support. This may also involve assistance with comfort devices and safety measures.
• Observe and report patients’ physical, mental, and emotional conditions, noting any changes and reporting them to the primary nurse/charge nurse.
• Perform oral care, hair care, peri care, and catheter care.
• Weigh patients and document weight in the appropriate area.
• Answer call lights for all patients in a courteous and timely manner; position call lights within reach of the patient.
• Document and maintain accurate patient records and care activities.
• Maintain a clean and safe environment, change bed linens, and at times clean rooms, ensuring safety measures such as bed alarms are in place.
• Stoke supplies and replenish medical supplies in patient rooms and other areas.
• Assist with procedures, supporting nursing staff and/or provider as directed.
• Transport patients, using lift equipment safely, and assist with moving patients throughout the hospital for treatments, tests, procedures or activities, utilizing wheelchairs, stretchers, or beds.
