Job summary

Boonespring of Union in Kentucky is seeking a Skin Integrity Nurse for our state-of-the-art long term care facility. The RN must hold an active license in the state and be flexible with scheduling.

The role includes weekly head-to-toe skin assessments, documentation in the EHR, and collaboration with clinicians to adjust orders as needed. You will support wound care, update care plans, and participate in SIT meetings and QA activities.

Qualifications

• Must be a graduate of an accredited nursing program with current RN licensure in the state.
• Willing to learn about the integumentary system and continue education.
• Strong ability to perform weekly skin assessments and document in the EHR.

Responsibilities

• Must be a graduate of an accredited school of nursing with a current licensure as a Registered Nurse in the state where the facility is located.
• Complete weekly head to toe skin assessments and document in the EHR.
• Ensure appropriate treatments and interventions are in place for skin issues identified during skin rounds.
• Collaborate with Clinician or Wound NP to obtain or discontinue orders as needed.
• Update ADON and DON on ulcers and wound progress.
• Verify/update resident care plans and documentation for identified skin impairments.
• Monitor skin integrity practices and infection control procedures.
• Assist in evaluating and implementing QA recommendations.
• Provide interventions for residents at high risk for pressure ulcers and ensure updates to families.
• Attend SIT meetings and educate staff on wound care methods.

SKIN INTEGRITY NURSE

JOB DESCRIPTION$36.00 - $43.00 base pay!!

GENERAL CONSIDERATIONS:

• The Skin Integrity Nurse has a current RN license for the state in which he /she is working.
• The nurse is flexible with her / his working schedule.
• The position requires an understanding of the integumentary system and a willing to continue to learn.

RESPONSIBILITIES:

• Must be a graduate of an accredited school of nursing with a current licensure as a Registered Nurse in the state where the facility is located.
• The Skin Integrity Nurse will complete weekly head to toe skin assessments and document in the EHR.
• The Skin Integrity Nurse will ensure that appropriate treatments and/or interventions are in place for skin issues identified during the skin rounds.
• The Skin Integrity Nurse will collaborate with the Clinician or Wound NP to obtain new orders or discontinue orders as needed.
• The Skin Integrity Nurse will update the ADON and DON if any new or worsened ulcers are identified.
• The Skin Integrity Nurse will update the ADON and DON with any a non healing wounds.
• The Skin Integrity Nurse will verify/update resident care plans and update documentation for identified skin impairments.
• The Skin Integrity Nurse will monitor skin integrity practices and procedures to ensure that personnel are implementing the facility’s SIT program.
• The Skin Integrity Nurse will demonstrate proper infection control practices.
• The Skin Integrity Nurse will assist in evaluating and implementing recommendations from the Quarterly QA meeting.
• The Skin Integrity Nurse will ensure that residents with known non compliance behaviors or are at high risk for pressure ulcers are provided with interventions as appropriate.
• The Skin Integrity Nurse attends and participates in weekly SIT meetings.
• The Skin Integrity Nurse will ensure residents’ responsible parties are updated on wound progress.
• Proactively develops positive employee relations and promotes teamwork, mutual respect and effective communication
• Education of new and inexperienced nurse aides in proper methods of patient care.
• Performs other duties as requested by the DON or ADON
• Possess and demonstrate a high degree of leadership, organizational ability and communication skills
• Must be able to utilize excellent communications skilled to relate to patients, families, and staff members
• He/she must also constantly update his/her knowledge through ongoing education.
