Job summary

Boonespring in Union, KY is seeking a Skin Integrity Nurse (RN) to join a state-of-the-art long-term care facility. The role focuses on weekly skin assessments, documenting in the EHR, and coordinating care with clinicians to prevent and manage skin issues.

A current RN license and a willingness to learn are required. This full-time position supports resident wellness and collaborative care within the SIT program.

Qualifications

• Must have current RN license in the state of employment.
• Graduate of an accredited school of nursing.
• Strong knowledge of the integumentary system.
• Willingness to continue learning.
• Ability to document in EHR and coordinate care.

Responsibilities

• Perform weekly head-to-toe skin assessments and document in the EHR.
• Ensure appropriate treatments/interventions for skin issues identified in skin rounds.
• Collaborate with Clinician or Wound NP for orders as needed.
• Update ADON and DON on ulcers or non-healing wounds.
• Update resident care plans for identified skin impairments.
• Monitor SIT program compliance and infection control practices.
• Assist in QA recommendations and resident risk interventions.
• Educate new/existing aides on proper patient care methods.
• Perform other duties as requested by DON or ADON.

SKIN INTEGRITY NURSE

JOB DESCRIPTION$36.00 - $43.00 base pay!!

GENERAL CONSIDERATIONS:

• The Skin Integrity Nurse has a current RN license for the state in which he /she is working.
• The nurse is flexible with her / his working schedule.
• The position requires an understanding of the integumentary system and a willing to continue to learn.

RESPONSIBILITIES:

• Must be a graduate of an accredited school of nursing with a current licensure as a Registered Nurse in the state where the facility is located.
• The Skin Integrity Nurse will complete weekly head to toe skin assessments and document in the EHR.
• The Skin Integrity Nurse will ensure that appropriate treatments and/or interventions are in place for skin issues identified during the skin rounds.
• The Skin Integrity Nurse will collaborate with the Clinician or Wound NP to obtain new orders or discontinue orders as needed.
• The Skin Integrity Nurse will update the ADON and DON if any new or worsened ulcers are identified.
• The Skin Integrity Nurse will update the ADON and DON with any a non healing wounds.
• The Skin Integrity Nurse will verify/update resident care plans and update documentation for identified skin impairments.
• The Skin Integrity Nurse will monitor skin integrity practices and procedures to ensure that personnel are implementing the facility’s SIT program.
• The Skin Integrity Nurse will demonstrate proper infection control practices.
• The Skin Integrity Nurse will assist in evaluating and implementing recommendations from the Quarterly QA meeting.
• The Skin Integrity Nurse will ensure that residents with known non compliance behaviors or are at high risk for pressure ulcers are provided with interventions as appropriate.
• The Skin Integrity Nurse attends and participates in weekly SIT meetings.
• The Skin Integrity Nurse will ensure residents’ responsible parties are updated on wound progress.
• Proactively develops positive employee relations and promotes teamwork, mutual respect and effective communication
• Education of new and inexperienced nurse aides in proper methods of patient care.
• Performs other duties as requested by the DON or ADON
• Possess and demonstrate a high degree of leadership, organizational ability and communication skills
• Must be able to utilize excellent communications skilled to relate to patients, families, and staff members
• He/she must also constantly update his/her knowledge through ongoing education.
