Job summary

Prisma Health in Greenville Memorial Hospital seeks a registered nurse to coordinate an interdisciplinary plan of care for the patient population served. This role functions as a clinical, patient, and community resource, educator, and consultant.

You will participate in direct and indirect patient care, support quality improvement, document accurately, and contribute to community education. An Associate degree in Nursing is required; a Bachelor’s is preferred; three years of nursing experience

Qualifications

• Education: Associate degree in Nursing; Bachelor’s degree preferred.
• Experience: Three (3) years of nursing experience.
• Licensure: Licensed to practice as a Registered Nurse in South Carolina.

Responsibilities

• Coordinate an interdisciplinary plan of care for patient populations served.
• Participate in direct and indirect patient care, rounds, and multidisciplinary planning.
• Collaborate with health team to develop and evaluate quality programs.
• Develop, provide, and evaluate educational programs for patients, families, staff, and the community.
• Monitor documentation, reports, and statistics related to the program.
• Support quality of care through ongoing process improvement.
• May participate in research/investigative studies when indicated.
• Perform other duties as assigned.
