The Nurse Practitioner – Wound Care plays a critical role inassessing patients. This position requires a wound care certified nursepractitioner who is experienced, team-oriented, and able to provide expertwound care in the office or nursing home setting according to the practiceguidelines of the State of Tennessee or Mississippi. Qualified candidates willhave strong knowledge of evidenced-based wound care practices, the use of skinsubstitutes and other strategies to improve targeted outcomes. The certifiedwound care nurse practitioner will work closely with the assigned team tomonitor outcomes, clinical efficiency, appropriateness/quality of care andsupervise care.

Duties and Responsibilities:

• Demonstrates proficiency in performing the full scope ofwound care and management, including patient health interviews, review ofmedical/surgical health history, physical assessment, ordering andinterpretation of diagnostic testing, diagnosing health problems, managinghealth care, providing health teaching and counseling, initiation of referrals,following all appropriate guidelines for indicated treatment and follow-upscheduling, collaborating with assigned team members and other health care providers,and maintaining electronic health records.
• Complete documentation that meets regulatory guidelinesand ensures compliance with documentation in the application-based electronicpatient record for clinical care instructions, communications, wound caresupply orders, and diagnostic testing results.
• Prescribes appropriate treatment orders in accordance withthe guidelines for wound dressings, laboratory testing, cultures, imaging,specialty/offloading, and pressure reducing devices.
• The advanced practice nurse may prescribe non-scheduledlegend drugs, initiative, monitor, alter or order drug therapies based oneducational preparation, clinical experience, and state guidelines/scope ofpractice either in collaboration with a physician provider or under thesupervision of a physician provider.
• Implement and maintain professional patient communicationsduring in-home visits and via the application-based electronic patient record.
• Maintains and ensures regional compliance with allregulatory and company standards.
• Completes and monitors appropriate notices for dischargeincluding referral to the case manager for post-healing follow-up within theassigned region when indicated.

Minimum Qualifications:

• Current State Nursing License as an APRN or Physician Assistant in Tennessee and/or Mississippi.
• Certification as a Family Nurse Practitioner throughthe AANP or ANCC or Certification as a Physician Assistant through theNCCPA.
• DEA license, or willingness to obtain upon hire.
• Certification in wound care (CWCA, CWCN, orequivalent)
• Minimum of two (2) years of experience in wound caremanagement, with proficiency in wound assessment.
• Proficient in debridement method(s) required.
• Strong interpersonal and communication skills, withthe ability to effectively collaborate with patients, families, and teammembers.

Physical Requirements:

The physical activities of this position may includeclimbing, pushing, standing, hearing, walking, reaching, grasping, kneeling,stooping, and repetitive motion. Must have good balance and coordination. Thephysical requirements of this position are medium work - exerting up to 50 lbs.of force occasionally and/or up to 25 lbs. of force frequently. The NursePractitioner – Wound Care is required to have close visual acuity includingcolor, depth perception, and field of vision to perform an activity, such asassessing patients, and analyzing data and figures; transcribing; viewing acomputer terminal; or extensive reading. Frequent invasive and non-invasivepatient contact. Exposure to patient body fluids as well as exposure tohazardous or poisonous materials.
