Qualifications:
- At least four total licenses are required for this position.
- Must be licensed in at least one of these states: AZ, IL, CA, MO, MN.
- Must ALSO be licensed in at least three of these other states: AK, AZ, CA, HI, IL, MO, MN, MT, NM, OR, UT, WA.
The Telehealth Nurse Practitioner or Physician Assistant (Provider) delivers patient care services through a remote technology platform. You will work in collaboration with a dedicated team of professionals, independently providing holistic, evidence-based care including assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for patients aged 18 months and above. Encounters are documented using an electronic health record (EHR). Telehealth providers report directly to the Enterprise Initiative Lead.
Responsibilities:
- Evaluate primary care, acute, chronically ill, and transitional care patients, providing healthcare education, counseling, and disposition planning.
- Provide patient counseling, including pregnancy prevention, STI prevention/safer sex practices, contraceptive care, and medication management.
- Educate patients on health maintenance and respond to inquiries.
- Document all patient care within an EHR according to policies and procedures.
- Coordinate care with internal and external colleagues, including the broader patient-centered medical home.
- Work effectively within a patient care team, including Providers, Physicians, para-professionals, Pharmacists, and others.
- Enhance patient engagement and satisfaction through feedback integration.
- Create a warm, welcoming environment and tailor communication to patient needs.
- Collaborate with pharmacy and front-store colleagues for a complete patient experience.
- Maintain patient confidentiality per PHI and HIPAA standards.
- Foster positive relationships among practice staff and external partners.
- Reprioritize tasks throughout the day to meet patient and business needs.
- Support the overall patient experience by managing clinical and non-clinical duties.
- Adopt new service offerings and care models promptly.
- Adhere to core values in all interactions.
- Assist with hiring, development, and evaluation of practice staff.
- Handle clinic operations such as inventory, lab follow-ups, calls, supplies, and cleanliness.

- Manage business needs including budget, payroll, inventory, billing, and collections.
- Implement practice business plans to meet goals.
- Ensure complete and accurate payment documentation and collection.
- Perform revenue cycle management tasks.

- Work independently, prioritize, solve problems, and advocate for patients and practice.
- Engage in professional development through external and internal groups.
- Maintain professionalism and self-awareness, understanding the impact of actions.
- Complete ongoing education to maintain licensure and employment.
Include Nurse Practitioner Questionnaire on resume. Must be willing/available to work two weekends per six-week cycle and one major and one minor holiday per year.
Experience Requirements:
- Minimum of two years of relevant medical experience.
- High-speed internet with at least 25 Mbps download and 3 Mbps upload speeds.
- Unrestricted license with multi-state or compact privileges as required.
- Effective communication skills, organizational skills, initiative, and adaptability.
- Ability to work autonomously and collaborate effectively.
- Proficiency with information management and technology.
Completion of a Master’s Degree in Family Nurse Practitioner program, current certification, and state licensure are required.
