Job summary

The Nurse Practitioner at the Manchester VA Office of Community Care (OCC) is a full-time position providing direct primary care to Veterans, including health assessments, diagnosis and management of acute and chronic conditions, and prescription of medications.

The role coordinates care with community providers, reviews consults, and ensures thorough follow-up. It also involves educating Veterans and families about VA resources and health promotion options.

Responsibilities

• Provides direct primary care to Veterans, including health assessments, diagnosing and managing acute and chronic conditions, and prescribing medications.
• Coordinates and manages the care of Veterans referred to community providers, including reviewing consults, authorizing services, tracking appointments, and ensuring follow-up.
• Acts as liaison between Veterans, VA staff, and community care partners to facilitate communication, care transitions, and continuity of care.
