Job summary

VA Black Hills Health Care System seeks an Advanced Practice Nurse/Certified Nurse Practitioner to deliver primary care services to Veterans in Hot Springs, SD. The role emphasizes health promotion, disease prevention, and collaborative care within the Primary Care Service Line.

The NP will manage a patient panel, order labs and referrals, and provide patient and family education while upholding ICARE values. Eligible for Education Debt Reduction Program (EDRP) assistance.

Qualifications

• APN/NP certification required.
• Experience in primary care setting preferred.
• Ability to order labs, tests, and diagnostic studies; make referrals.

Responsibilities

• Independently manages a panel of patients with acute and chronic conditions.
• Develops and implements individualized plans for health promotion and disease management.
• Demonstrates leadership and collaborates with the Primary Care Team and interdisciplinary colleagues.
• Orders and interprets labs, tests, and diagnostics; prescribes interventions.
• Provides education to patients and families; adjusts plans to improve outcomes.
