Job summary

Qualifications

• APN/NP licensure required in any US state or territory.
• Advanced clinical practice skills in primary care.
• Ability to develop and manage treatment plans using evidence-based practice.

Responsibilities

• Independently manages a panel of patients with acute and chronic health conditions with emphasis on health promotion and disease prevention.
• Provides advanced clinical practice, consultation, and management in primary care.
• Orders and interprets labs, tests, and diagnostic studies.
• Prescribes pharmacologic and non-pharmacologic interventions.
• Initiates referrals and educates patients and their families.
