Qualifications

- 6 Months nurse practitioner experience in Primary Care/Internal Medicine.
- Value-based care experience preferred.
- A reliable automobile, valid driver’s license required.
Responsibilities

- Gather patient history and conduct physical exams.
- Collaborate with interdisciplinary teams for optimum patient care.
- Provide compassionate care to complex patients and their families.
- Educate patients about medication and nutrition.
