Job summary

Heritage Urgent and Primary Care, a division of Raleigh-Durham Medical Group, seeks a Nurse Practitioner to join our growing clinical team serving the Wake Forest and Raleigh communities. Qualifications

• Graduate of an accredited Physician Assistant program (Master’s Degree required).
• Licensed or eligible for licensure as a Physician Assistant in North Carolina.
• 2+ years of clinical experience preferred.
• Strong clinical decision-making and patient management skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced outpatient environment.
• Proficiency with electronic health record (EHR) systems preferred.

Responsibilities

• Provide comprehensive medical care for patients in both urgent care and primary care settings.
• Conduct patient evaluations including medical histories, physical examinations, and diagnostic assessments.
• Diagnose and manage acute and chronic medical conditions.
• Order and interpret laboratory tests and diagnostic studies.
• Prescribe medications and develop individualized treatment plans.
• Provide patient education regarding treatment, prevention, and wellness strategies.
• Document all patient encounters accurately in the electronic health record (EHR) system.
• Collaborate with physicians, advanced practice providers, and clinical staff to ensure continuity of care.
• Participate in quality improvement initiatives and clinical protocols.
