Trinity Health PACE in Delaware seeks a Nurse Practitioner to join our team, delivering comprehensive primary medical management in collaboration with our PACE Organization physicians. You will conduct assessments, develop care plans, and ensure participants receive high-quality, patient-centered care in a warm, supportive environment.
Responsibilities include collaborating with PO physicians, performing assessments, discharge planning, and home visits as needed.
