Job summary

WellBe Senior Medical is seeking a Nurse Practitioner (APP) to deliver field-based primary care in patients' homes in Charleston, WV. This role offers autonomy, a strong interdisciplinary team, and a $20K sign-on bonus.

You will manage chronic disease, perform comprehensive exams, and coordinate care to prevent ER visits, with relocation and full benefits. Active WV NP license and ANCC/AANP board certification are required.

Qualifications

• Master’s Degree: NP with Acute Care, Adult, Geriatric or Family focus.
• WV NP license active and board certification (ANCC or AANP).
• Valid driver’s license and vehicle for local field-based travel.

Responsibilities

• Deliver Field-Based Care: conduct comprehensive primary care visits and exams in the patient’s home.
• Manage Chronic Disease: create longitudinal care plans for high-risk patients.
• Collaborate Across the Continuum: partner with an interdisciplinary team to ensure safe care transitions.
• Prevent ER Visits: treat acute needs in the home to keep seniors safe and out of the hospital.

• Advanced Degree: Master’s Degree (NP) with a preferred focus on Acute Care, Adult, Geriatric, and/or Family.
• WV Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP).
• Reliable Mobility: Valid driver’s license and vehicle for local field-based travel.
