Riddle Village, a lifecare retirement community, is seeking a dedicated Nurse Practitioner to provide quality care in our Skilled Nursing/Long Term Unit and Personal Care Unit.

You will collaborate with NPs, Therapists, Nurses and Physicians to deliver comprehensive care, perform examinations, order tests, and coordinate treatment plans while ensuring accurate EMR documentation and clear communication with residents' families.

Qualifications

• Current and unrestricted Pennsylvania State NP License.
• Current and unrestricted DEA License.
• Desire to work in subacute rehabilitation, long-term care and personal care.
• Outstanding written and verbal communication skills.
• Ability to document decisions, risks, and treatment plans clearly and effectively.
• Proficiency in using electronic medical records systems.
• Ability to work independently and collaboratively in our healthcare setting.
• Commitment to continuing education and professional development.

Responsabilités

• Provide care to Subacute Rehab and Long-Term Care residents on our Skilled Nursing Unit and our Personal Care Unit.
• Collaborate with other NPs, Therapists, Nurses and Physicians to provide quality care.
• Examine residents and develop a treatment plan in coordination with other disciplines.
• Order and interpret diagnostic tests.
• Prescribe appropriate medications and therapies.
• Ensure proper documentation in our Electronic Medical Record.
• Ensure that residents and their families are kept apprised of clinical progress during their stay.
