Responsibilities

• Serves in the role of primary care provider to long and short term residents; assesses health by visiting patients, performing physical exams, updating histories.
• Collaborates with the medical attending/physician and nursing home staff to provide high-quality care.
• Educates residents and families, and formulates evidence-based care plans aligned with patient needs and desires.
• Complies with federal, state, and local requirements and advises management on needed actions.
• Maintains professional knowledge by attending workshops and reviewing literature.
• Ensures a safe, clean working environment and aligns with organizational values.
• Reviews charting and documentation to ensure timely and accurate care administration.
