Job summary

Avir Health Group is seeking a Nurse Practitioner (Skilled Nursing) to join our team in a leadership role within a Skilled Nursing Facility. You will oversee admissions, discharges, and monthly stability checks, and conduct evaluations for changes in condition while coordinating with the facility care team.

The ideal candidate holds an active Texas APN license and DEA registration, has nursing/clinical experience, and demonstrates strong communication and teamwork skills.

Qualifications

• Active Texas Advanced Practice Licensure (APN) and DEA license.
• Previous nursing or clinical experience.
• Strong communication and teamwork skills.

Responsibilities

• Manage admissions and discharges for SNF residents.
• Perform monthly stability checks and evaluations for changes in condition.
• Oversee advanced care planning and comprehensive patient assessments.
• Prescribe treatments as appropriate within scope of practice.
