Aviata Health Group is seeking a qualified Nurse Practitioner (Skilled Nursing) to join our family in a SNF setting, where you will serve as a clinical leader and collaborate with the facility's care team to oversee both short- and long-term residents. Admissions, discharges, monthly stability checks, and evaluations for changes in condition are part of the role.

This role requires Florida APN licensure and DEA license, a Master’s in Nursing, and strong communication and teamwork.

Qualifications

• Active Florida Advanced Practice Licensure (APN) and DEA License.
• Master's degree in Nursing is required.
• Experience in nursing/clinical settings preferred.

Responsibilities

• Oversee admissions and discharges of SNF residents.
• Perform monthly stability checks and assessments for changes in condition.
• Lead advanced care planning and comprehensive patient assessments.
• Prescribe treatments in coordination with the care team.
