Avir Health Group is seeking a Nurse Practitioner (Skilled Nursing) to join our clinical team in a SNF setting, acting as a clinical leader with the facility’s care team to oversee residents across short- and long-term stays.

You will oversee admissions, discharges, monthly stability checks, and changes in condition, while handling advanced care planning, comprehensive assessments, and prescribing treatments in collaboration with physicians.

Qualifications

• Active Texas Advanced Practice Licensure (APN) and DEA License
• Previous nursing or clinical experience
• Strong communication and teamwork skills
• Master’s degree in Nursing

Responsibilities

• Admit and discharge SNF residents.
• Perform monthly stability checks and assess changes in condition.
• Lead advanced care planning and comprehensive assessments.
• Prescribe treatments in collaboration with physicians.
