You’ll manage admissions and discharges, conduct monthly stability checks, perform evaluations for changes in condition, and lead advanced care planning and comprehensive assessments, with prescribing responsibilities.

Qualifications

• Active Texas Advanced Practice Licensure (APN) and DEA License.
• Previous nursing or clinical experience.
• Strong communication and teamwork skills.

Responsibilities

• Oversee admissions and discharges in SNF.
• Conduct monthly stability checks and evaluations for changes in condition.
• Perform comprehensive assessments and develop advanced care plans.
• Prescribe treatments as appropriate.
