Job summary

You will handle advanced care planning, comprehensive assessments, and prescribing treatments to support residents’ health and quality of life while promoting safety and teamwork among staff.

Qualifications

• Active Texas APN licensure required.
• DEA license required or eligible for DEA.
• Master’s degree in Nursing with activeNP certification.

Responsibilities

• Manage admissions and discharges for SNF residents.
• Perform monthly stability checks and evaluations for changes in condition.
• Develop and implement advanced care plans.
• Conduct comprehensive assessments and prescribe treatments as appropriate.
