Job summary

CHS Recruiting seeks a Nurse Practitioner for a Skilled Nursing Facility in Terre Haute, IN. This full-time role features Monday–Friday schedules with no evenings, weekends, or holidays, and a light on-call burden. You will provide primary care to geriatric residents in a low-volume setting using mobile EHR support.

Starting base salary ranges from $115,000 to $130,000+ and includes a $10,000 sign-on bonus, plus a generous benefits package and 401k with match. Negotiable based on experience.

Qualifications

• Indiana NP license required.
• DEA registration required.
• 1+ year advanced practice experience.
• 1+ year experience in facilities/home care/geriatrics.

Responsibilities

• Plan, direct, and evaluate medical care.

• Order and interpret laboratory and diagnostic testing.
• Collaborate with caregivers and other healthcare professionals.

POSITION DESCRIPTION

Serve as a primary care provider for geriatric patients located in a skilled nursing facility. A low volume of patients are seen per day, allowing ample time to focus on all needs and to provide proper attention and support.

Overall duties of the role include, but are not limited to:

• plan, direct, and evaluate medical care
• examine and treat chronic illnesses
• examine and treat urgent illnesses
• order and interpret laboratory and diagnostic testing
• collaborate with caregivers and other healthcare professionals
• work closely with the interdisciplinary care team
• make referrals as necessary

The practice uses an EHR on a mobile-based platform and providers are supported by team members who assist with scheduling, orders, referrals, etc.

Providers share on-call responsibilities, which is low volume phone triage. Providers are on call no more than 3 nights per month and flexibility in that schedule is offered.

REQUIREMENTS

• 1+ year advanced practice experience
• 1+ year experience in relevant settings (facilities, home care, geriatrics, etc.)
