The Geisinger Skilled Nursing Program is seeking an experienced Nurse Practitioner for the Embassy of Wyoming Valley, Wilkes Barre, and Embassy of Scranton, Scranton, PA.

Job Duties

This position is as a Geisinger provider in a non-Geisinger facility and requires an experienced advanced practitioner, with prior experience working in a skilled nursing facility, family medicine, internal medicine, or ICU. The advanced practitioner (AP) provides direct (hands-on) care in skilled nursing facilities with the goal to improve quality and prevent unnecessary re-admissions to the acute care setting.

Responsibilities

• Review reports
• Communicate with SNF staff for patient needs
• Treat patients for acute needs, admissions, discharges, etc.
• Manage extremely complex patients, see every day or at least once a week
• See approximately 12 patients a day
• Provide care based on patient needs, including length of time per patient and number of patients
