Qualifications

• Active ANCC/AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Current, unrestricted state licensure (or ability to obtain by start date).
• Active DEA licensure/prescriptive authority or ability to obtain post-hire.

Responsibilities

• Conduct comprehensive assessments: Hx, meds, vitals, exam.
• Coordinate care and documentation across care settings.
• Participate in advanced illness and end-of-life planning conversations.
• Collaborate with interdisciplinary care team to meet patient needs.
