Curana Health is seeking a Nurse Practitioner or Physician Assistant to lead primary care for seniors across communities. You’ll provide proactive, relationship-based care in residential settings, traveling to facilities as needed.

The role emphasizes autonomous practice, support from physicians and operations, and a technology-driven approach to improve outcomes and reduce hospitalizations.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification in your field.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and teamwork ability.

Responsibilities

• Deliver high-quality primary care in skilled nursing, assisted living, and independent living communities.
• Perform comprehensive health assessments and develop individualized treatment plans.
• Diagnose, treat, and manage acute and chronic conditions with focus on preventative care.
• Build meaningful patient relationships through regular rounds and family collaboration.
• Collaborate with physicians, staff, and families to coordinate care plans.
• Maintain accurate EMR documentation and ensure regulatory compliance.
