Job summary

Community Health Centers of the Rutland Region seeks a Nurse Practitioner to join the Castleton Express Care clinic on a per diem basis. This Express Care role emphasizes timely assessment and treatment with responsibilities including histories, exams, orders, prescriptions, and patient education.

Eligible candidates must be Vermont-licensed NPs or PAs with 3–5 years in express/urgent care, excellent teamwork, and strong organizational and communication skills.

Qualifications

• 3–5 years Express/Urgent Care practice experience.
• Graduate of an accredited Acute Care NP or PA program.
• Licensed for Nurse Practitioner or Physician Assistant in Vermont.
• Quality-focused team player with strong organizational skills.

Responsibilities

• Take complete medical histories and perform physical exams.
• Interpret data and determine diagnostic/therapeutic plans.
• Order labs and diagnostic procedures as needed.
• Develop therapeutic plans and write prescriptions.
• Instruct patients/families on meds and treatment.
• Provide health maintenance recommendations and community resources.
• Perform evaluations in emergencies and ensure continuity of care.

Community Health Center sites are certified Level 111 Medical Homes, with a highly developed and supportive panel management and case management system in place. Community Health also has a robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed Clinical Social Workers embedded within its primary care sites. We have a fully integrated electronic medical record and practice management system (Medent) with robust interfaces with our local hospital.

POSITION SUMMARY:

Community Health is seeking a Nurse Practitioner to work at our Castleton Express clinic. Candidates are expected to be knowledgeable and skilled in the delivery of express care. Responsibilities include, but are not limited to:

• Obtains complete medical history and physical examination on patients.
• Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures.
• Orders appropriate laboratory and diagnostic procedures.
• Develops a therapeutic plan for patient.
• Prepares written prescription orders for drugs and controlled substances.
• Instructs patients and family regarding medications and treatment instructions.
• Provides recommendations regarding health and illness prevention.
• Recommend community resource to meet patient/family needs.
• Performs independent evaluations and treatment procedures in emergency situations.
• Provides monitoring and continuity of care between physician visits.

KNOWLEDGE, EDUCATION AND SKILLREQUIREMENTS:

• 3-5 years Express / Urgent Care Practice Experience.
• Graduate of an accredited Acute Care Nurse Practitioner or Physician Assistant Program.
• Licensed for Nurse Practitioner or Physician Assistant license in the state of Vermont.
• Quality-focused team player.
• Excellent organizational, time management and problem-solving skills.
• Ability to function effectively as part of a multi-disciplinary team.
• Effective oral and written communication skills.
• Commitment to providing exceptional patient care.
