Job summary

Centre de santé Univi Health Centre is seeking a qualified Registered Nurse to provide primary health care across ages in a CHC model, including assessments, management, and education. You will work with an interdisciplinary team to deliver coordinated patient care and participate in health promotion initiatives.

The role requires BSc in Nursing, RN-EC, and ideally a Master's in Nursing, with CPR/First Aid; French/English bilingualism is preferred; condensed four-day work week and HOOPP benefits

Qualifications

• Bachelor of Science in Nursing (RN) required.
• Registered Nurse in the Extended Class (as recognized by CNO).
• Masters in Nursing preferred.
• Preference for experience in a community health or primary care setting.
• CPR and First Aid certification required.
• Knowledge of health promotion, education and program evaluation.
• Knowledge of community resources and social services.
• Ability to work within an interdisciplinary team.
• French and English bilingual preferred.
• Proficiency with computers and digital communication tools.

Responsabilities

Provide primary health care services to patients of all ages in a CHC model of care including health assessments and diagnosis, health care management and therapeutic interventions, screening, referrals, education, follow-up as well as health promotion and the prevention of illness and injury in accordance with the Standards of Practice for Nurse Practitioners.

Collaborate with members of an interdisciplinary team to plan, implement, coordinate and evaluate patient care in consultation with patients and their families.

Participate in the development, delivery and evaluation of educational and health promotion programs within the community as needed.

Qualifications

Work Conditions
