WellBe Senior Medical is seeking a Nurse Practitioner (APP) for field-based primary care in Rochester, NY. You will conduct comprehensive home visits, manage chronic conditions, and coordinate with a multidisciplinary team to ensure safe transitions and high-quality care for seniors.

The role emphasizes autonomy, ongoing mentorship, and a patient-centered approach to reduce ER visits and improve health outcomes. Relocation may be available for the right candidate.

Qualifications

• Active, unencumbered NP license.
• Reliable mobility: valid driver's license for local field-based travel.

• Manage Chronic Disease: Create longitudinal care plans for high‑risk patients.
• Collaborate Across the Continuum: Partner with an interdisciplinary team for safe care transitions.
• Prevent ER Visits: Manage acute needs at home to keep seniors safe and out of hospital.

Job Requirements

What You Bring:

• Advanced Degree: Master’s Degree (NP) with a preferred focus on Adult, Geriatric, Acute Care and/or Family.

• State Required Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP).

• Reliable Mobility: Valid driver’s license and vehicle for local field-based travel.
