TruHealth is a division of American Health Partners, operating in multiple states with nursing homes, health plans for Medicare beneficiaries, psychiatric care, home health, hospice, rehabilitation, and pharmacy services.

Position Summary

We are seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with experience in primary care or internal medicine, preferably with geriatrics focus. This role involves improving outcomes for long-term care residents through collaboration with a team of providers and case managers. As an advanced practice provider, you will exercise independent judgment to treat patients with acute and chronic conditions, consulting with a supervising physician as needed. You will work closely with facility partners and health plan leadership to implement our care model in compliance with regulations.

Duties

  • Assess patients’ medical and mental health needs and develop care plans
  • Ensure compliance with relevant regulations
  • Prescribe medications and order diagnostic tests
  • Monitor and modify treatment plans as needed
  • Collaborate with RN case managers within an integrated care team

Work Experience, Credentials, and Education

  • FNP, AGNP, AHACNP, or PA license required
  • Degree from an accredited APRN or PA program
  • Minimum 3 years’ experience in geriatric clinical nursing or rehab
  • Experience with Electronic Health Records
  • Proficiency in Microsoft Office applications

Supervisory Responsibilities

  • May include training and advising facility staff
