Job summary

Curana Health is seeking a Psychiatric Nurse Practitioner (PMHNP) to join our growing team and provide high-quality behavioral health services across SNF, LTC, AL, and IL communities. This is a full-time role requiring travel to assigned facilities and delivering comprehensive psychiatric care in a multidisciplinary setting.

You will work with a collaborative care team to deliver personalized services, empower residents, and improve outcomes for older adults in post-acute and long-term care

Qualifications

• Graduation from an accredited PMHNP program.
• ANCC certification as PMHNP.
• Active DEA license.
• Current Nurse Practitioner license in the state of practice.
• BLS and ACLS certification.

Responsibilities

• Provide psychiatric care and rounding in SNF, LTC, AL, and IL settings.
• Collaborate with attending physicians and facility staff to coordinate care.
• Evaluate residents, perform exams, and document medical histories.
• Order and interpret labs and tests; develop treatment plans.
• Prescribe medications and provide injections as needed.
• Educate patients and families on medications and care plans.
• Support acute care needs with the care team.

Essential Duties & Responsibilities

• Psychiatric Care and Rounding: Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings.
• Collaborative Care Coordination: Partner with attending physicians, consulting specialists, referral sources, and facility staff to ensure comprehensive, coordinated care.
• Patient Evaluation and Management: Assess and manage the mental health needs of residents, perform physical exams, and gather complete medical histories.
• Diagnostics and Therapeutics: Order and interpret lab and diagnostic tests, make preliminary diagnoses, and develop effective therapeutic care plans for mental and physical health.
• Medication and Treatment Management: Prescribe and administer medications and treatments; provide injections as necessary.
• Patient and Family Education: Educate patients and families on medications, care plans, and preventive mental health practices.
• Acute Care Collaboration: Work alongside physicians and facility teams to address acute psychiatric and medical needs promptly and effectively.

This role requires travel to and from one or more assigned facilities. Depending on the amount of travel required, Curana may in future determine that a valid driver’s license, current auto insurance, and an acceptable driving record are required.
