Hueman is recruiting Nurse Practitioners to conduct in-home Health Risk Assessments for Medicare and Medicaid members around Salisbury, MD. This role offers a flexible schedule and the autonomy to balance professional goals with personal needs.

Travel to patient homes is required, with scheduling support and liability coverage provided. As an NP, you will perform non-invasive assessments, review medical histories, and document thoroughly.

Qualifications

• Master's degree in nursing (NP) or commensurate experience.
• Active NP license in good standing.
• National board certification as a Nurse Practitioner.
• Current BLS certification; 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative care.

Position Summary

Qualified Nurse Practitioners interested inthispositionwill have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs to connect with patients on a more personal and effective level. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.

Nurse Practitioner Responsibilities
